If you are currently working and your employer is requesting paperwork that needs to be completed by your surgeon, you can:

Be sure to complete your portion of the paperwork before sending it in, and please send it to us as soon as possible. It can take up to 10 days for processing.
Do not bring this paperwork with you on the day of your surgery; it must be completed ahead of time.
